Manhattan Community District 1

Read the consent form first in Tribeca and the district
Live or work in Manhattan Community District 1 or Tribeca and have a consent form in front of you? Read it line by line before you sign, and use this page to plan your questions.
Apollo House is the local reference for the service names on this site. Nothing here speaks for the clinic.
What a district provider should tell you
Take these questions to any provider near you.
- Who performs the service? Get that person's title and license.
- Which product or device is used? Ask for its exact name and find its labeling.
- Which terms does the consent form ask you to accept? Mark every line you cannot explain in your own words.
- What does the follow-up schedule look like? Ask for dates.
- If something goes wrong, which phone number do you use? Save it in your phone.
Trace a health notice to its issuing agency
A health notice you are handed may name a city agency or a state one. For local background, read the news report on New York medical spas and the state's department of state and the state agency page on esthetics, which covers one license type. Then find the notice itself on the issuing agency's own site and read it in full.
Take questions about your own case to your own clinician.
